What is High-Altitude Pulmonary Edema (HAPE)?

High-Altitude Pulmonary Edema, commonly known as HAPE, is a severe and potentially life-threatening form of altitude sickness that occurs when fluid accumulates in the lungs.

When fluid builds up inside the lungs, it becomes difficult for the body to absorb enough oxygen, leading to serious breathing difficulties.

HAPE most commonly develops when climbers ascend too quickly to high altitude without giving the body enough time to acclimatize.

This condition requires immediate medical attention.


Common Symptoms of HAPE

Recognizing symptoms early can save lives.

Watch for these warning signs:

  • Shortness of breath, even while resting
  • Difficulty walking or exercising
  • Extreme fatigue and unusual weakness
  • Rapid heartbeat
  • Persistent dry cough
  • Chest tightness or congestion
  • Blue or gray-colored lips and fingernails
  • Severe lack of energy
  • Coughing up pink or frothy sputum

What Causes HAPE?

At high altitude, oxygen levels decrease.

As oxygen becomes limited, blood vessels inside the lungs narrow and create increased pressure. This pressure can force fluid into the air spaces of the lungs.

As fluid continues to build up, breathing becomes more difficult and oxygen levels drop further.

Without prompt treatment, HAPE can become life-threatening.


What Should You Do If You Suspect HAPE?

1. Descend Immediately

The most effective treatment for HAPE is descending to a lower altitude as quickly and safely as possible.

In many situations, descending approximately 1,000–1,600 meters can significantly improve symptoms. In severe cases, descent to the mountain gate may be necessary.

2. Use Supplemental Oxygen

  • 125 mg in the afternoon or early evening

Many climbers prefer taking the second dose earlier in the afternoon because Diamox may increase urination.

Children should only use medication under professional medical guidance.


2. Dexamethasone

Dexamethasone may be used under medical supervision for prevention or treatment of severe altitude illness.

It is sometimes used in cases involving serious altitude-related conditions, including neurological altitude illness.

People who cannot take Diamox due to allergies should consult their doctor for alternatives.


3. Nifedipine

Nifedipine may be used in certain cases of High-Altitude Pulmonary Edema (HAPE).

This medication should only be taken under professional medical supervision.
